Request Coverage

Scheduling Information:   Only dates specifically marked AVAILABLE on the calendar may be requested; all other dates are unavailable. Standard weekday relief coverage is billed at an 8-hour minimum. New Practice Scheduling:   New practices may request multiple future dates; however, additional dates will initially be held on a contingent basis pending a successful working relationship during the first two shifts. This allows both parties to ensure that practice protocols, workflow, and team dynamics are a good fit before committing to ongoing coverage. 

Additional Information:   Once we have agreed upon the shift details and terms, I will provide a tailored independent contractor agreement for review and signature. Requested dates are not considered confirmed or reserved until the signed agreement has been returned.
